Job Description: JSLP-3467 Home Care Nurse – Health Care Industry (Ernakulam)

Responsibilities may include but are not limited to, confirm your Job Descriptions with the Interviewer:

  • Patient Assessment and Care Planning:
    • Conduct comprehensive assessments of patients’ health status, including physical, emotional, and social factors.
    • Develop individualized care plans in collaboration with patients, families, and caregivers.
    • Implement nursing interventions to address patients’ healthcare needs and promote optimal wellness.
  • Medication Management:
    • Administer medications and treatments as prescribed by physicians.
    • Monitor patients’ responses to medications and report any adverse reactions or changes in condition.
    • Educate patients and caregivers on medication administration and side effects.
  • Wound Care and Procedures:
    • Provide wound care, dressing changes, and other nursing procedures as needed.
    • Assist with medical procedures, such as catheter care, tracheostomy care, and tube feedings.
    • Ensure proper hygiene and infection control practices are followed.
  • Patient and Family Education:
    • Educate patients and families on disease management, self-care techniques, and preventive measures.
    • Provide guidance on nutrition, exercise, and medication adherence.
    • Teach caregivers how to perform caregiving tasks safely and effectively.
  • Emotional Support and Counseling:
    • Offer emotional support and counseling to patients and families coping with chronic illness, disability, or end-of-life care.

Qualifications:

  • GDA/ANM/GNM/Caregivers/Those who are not completed B.sc Nursing.
  • Freshers can apply.
  • Strong clinical skills and knowledge of nursing principles and practices.
  • Proficiency in using electronic medical records and other healthcare technology.
  • Excellent communication and interpersonal skills.
